Position: Certified Unit Clerk (Full Time)
Description: MVH is looking for a certified full time unit clerk. Responsibilities include: 1. Answers telephone/intercom and records and delivers messages, distributes and directs visitors. 2. Functions as a liaison for the Nursing in interdepartmental communications, i.e., Dietary – notification of changes in diets. 3. Prepares requisition forms for laboratory and x-ray tests and procedures. 4. Prepares and assembles new charts, secures previous admission charts and records. 5. Enters temperatures, blood pressure and other data on graphic sheets as obtained from physicians’ or nurses’ records. 6. Transcription of various physician orders as supervised by charge nurse. 7. Description: The Medical Records Supervisor is primarily accountable for overseeing the day to day functions of the medical records staff and maintaining control of confidentiality of medical records to protect the legal interests of the hospital and physicians. Position: Registered Dietitian: Clinical Nutrition & Diabetes Dept.
Description: Summary of position: • Works with Director, physicians, multidisciplinary team, and Clinical Nutrition and Diabetes Department (CND) staff to achieve program objectives and enhance clinical outcomes. • Reviews medical orders for all inpatient diets and modifies diets per Clinical Privileges in the provision of MNT. • Participates in the communications plan established for the CND. Represents the CND and MVH to physicians, local and national organizations, and community groups. • Based upon hospital needs, identifies and utilizes available and appropriate community services to enhance the CND and MVH’s services. • Participates in design and implementation of standards as defined by The American Diabetes Association for Outpatient DSME and Joint Commission Advanced Certification for Inpatient Diabetes Care. • Participates in the coordination and development of nutrition and/or diabetes training sessions for hospital staff as needed. • Mentors other clinical staff members in provision of medical nutrition therapy, nutrition physical assessment, and outcomes management practice. • Follows the documentation and education requirements set forth by the DPP via CDC&P. • Perform other duties as assigned. Position: Breast Cancer Navigator RN - (Full-Time)
Description: Position Title: Breast Cancer Nurse Navigator Reports to: Senior Vice President, Nursing DEFINITION: Provides comprehensive breast cancer care from diagnosis through surgery and recovery as an outpatient. Functions as a liaison to provide the opportunity to breast cancer patients to access any or all available resources. Provides collection and analysis of data for the Breast Center and completes associated reports. To maintain the Cancer Survivorship Program which includes patients from diagnosis to follow-up. PRINCIPLE ACCOUNTABILITIES: • Assesses the personal needs and concerns of cancer patients and their families • Helps the patients and their families understand the Breast Cancer Treatment Program through education and emotional support • Provides information and referrals for the cancer patient to access any and or all available resources needed to enhance their recovery • Involves the coordination and implementation of all cancer programs that include but are not limited to Reach to Recovery, Look Good-Feel Better, Cancer Support Group and Cancer Screenings • Collaborates with local and regional agencies to provide opportunities for the cancer patient to enhance the healing process and connect with other cancer patients • Assures the monitoring of outcomes for the Cancer Program; collects data that includes the demographics, the number of patients served, the number of referrals to resources, and the number of patients requiring financial assistance or transportation, etc. • Functions as a resource for physicians, nursing staff, various departments and the community Maintains communication with the cancer patients to check progress through the course of their experience to help assure their needs are being met • Coordinates the volunteers who are cancer survivors to assist with the emotional support of the patient as well as assist with Cancer Awareness Programs and screenings • Functions as an advocate as needed regarding insurance or financial issues • Is accountable to maintain current knowledge of cancer treatments and options, resources and other variables that impact the care of the cancer patient • Communicates with referring physician’s offices as required by the patient and physician • Participation in community and hospital events to educate other health professionals and the community on cancer diagnosis and treatment • Identifies development needs both immediate and projected, and prepares a self-development plan • Collaborates with the Marketing Department to arrange for program awareness and publicity • Performs professional responsibilities in an independent manner. Position: Maintenance Supervisor
Description: We are looking for a full-time Maintenance Supervisor who will be responsible for supervising and coordinating maintenance department personnel and activities to provide timely and effective maintenance and repair of all hospital physical facilities, plumbing, heating, air conditioning, electrical equipment and wiring, woodwork, furniture, equipment and grounds under the direction of the Director and Assistant Director of Engineering & Maintenance. Description: Cardio-Pulmonary Rehabilitation Nurse Function: Coordinates, executes the intake assessment/planning/implementation/evaluation/documentation of patient’s physical, psychosocial, and educational status during exercise, relaxation training, and all other therapeutic contacts made through participation during all stages of rehabilitation for the adult population. Position: CCN Care Coordinator, RN
Description: MVH is seeking a Care Coordinator (Full-Time) to ensure continuity and coordination of care across transitions, including hospital or facility to home; will identify barriers and facilitate optimal care delivery, including addressing gaps, management of medical conditions, medication access and management as well as psychosocial needs. The Care Coordinator will develop a comprehensive care plan to address those needs that include disease prevention, patient self-management, assessing and addressing social determinants of health, accessing and collaborating with community resources, and educating patients and their support systems about chronic disease management. The Care Coordinator will participate in community events and new initiatives that support and engage community members/programs to provide new support systems for the patients and families of Mon Valley. The Care Coordinator will follow patients into their home or other post-acute environment for monitoring and continuation of care. QUALIFICATIONS/EDUCATION REQUIREMENTS: Valid PA RN licensure, BSN Graduate, and NLN Approved School of Nursing, preferred.
